Pharmacist
Current• Reviews and evaluates medication orders by authorized practitioners and dispenses medication as per established procedure. Monitors drug therapy, maintains patient profiles, screens for potential drug interactions, allergies, and assures that appropriate dosages are prescribed.• Consults with physician and other health care professionals to discuss drug therapy regarding medication availability, formulary status, recalls, warnings, contraindications, interactions and incompatibilities. Provides drug information upon request and documents interventions accordingly.• Participates in Continuing Education Programs to keep abreast of the newest trends in drug therapy and to keep updated on the laws and regulations pertaining to pharmacy practice.• Maintains controlled drug inventory and assures accurate record keeping.• Reviews and evaluates monthly “trigger” medications to determine if there were any true adverse drug reactions (ADRs).• Prepares parental medications using aseptic technique in compliance with USP 797 and Joint Commission Standards. Document hood cleanings into Simplifi for each shift change. Complete competency in annual garbing and gowning fingertip test and media fill test. • Assist in the training for new technicians, interns, and pharmacists.• Assisted in the transition of an outdated clean room to a new ante and buffer room in compliance to USP 797 concerning gloving, gowning, and preparation of intravenous admixtures under the laminar flow hoods.
