- Responsible for engaging the youth and family in the care management service and CFT (Child Family Team) process and developing and facilitating the CFT meetings to ensure assessment & planning for youth and family’s needs.- To uphold the agency’s commitment to care coordination and provide strategies, supports, and services that are family centered, community based, strengths based, individualized, culturally competent, and easily accessible- To engage with youth and families by providing quality service delivery inclusive of assessment, development, referral, and monitoring duties.- Engage in assessment activities by listening to the family story and assessing for strengths, interests, and talents within the family.
