Worked as a nationally certified CPR & First Aid Instructor for both Community and Professional Rescuers. Primary responsibility is to facilitate classes of up to 25 persons with the end goal of the customer receiving their certification in CPR & First Aid. Also responsible for instructional design and keeping curriculum current. Other duties include marketing in person, via email, and by telephone to potential and past clients. Meeting with clients in person and evaluating the needs for their business and helping them to select the appropriate package was also a key responsibility. Other duties included the development of marketing materials, community resource networking, facility budgeting, and overall operating cost management while supervising a team of 6 people.

Co-Managed a team of 25-30 patients in their private homes and facilities providing hospice and palliative care alongside my Registered Nurse Partner. Primary duties included conducting 5-10 patient visits per day and assessing patients in their home. Ensuring the patient and family needs were met by providing comfort and pain care and coordinating with the pharmacy for refills of medications. Actively participated in interdisciplinary team meetings reviewing the overall progress of patients with the medical director. Other duties included developing plans of care, creating schedules for the nursing team including CNA’s, coordinating services for patients such as durable medical equipment and public assistance applications, and provide training in hospice education and philosophy to new employees; specifically training them on the proper use of the electronic health record.

Brought in to create, test, implement, and manage a new workflow process regarding patients facing solid organ and hematopoietic transplants. Served as Project Manager on several key programs including research and development, piloting a telecommuting program, writing and implementation of new policy and procedure, prior authorization guidelines specific to transplant patients, and served as community liaison/educator on behalf of the plan. Other key projects involved with include creation, testing, and implementation of a digital paperless work flow process and collaborating with Claims and Finance departments to recuperate $5+ million dollars in lost reinsurance. Also served as a mentor and trainer for new case and disease management staff teaching company specific programs, Microsoft Office, and Inter-Qual and Milliman standards. Also responsible for random audits to verify compliance with AHCCCS Medical Policy Manual (AMPM) and Centers for Medicare and Medicaid Services (CMS).

Assisted members in cost effective care coordination for the “at risk” population currently enrolled in AHCCCS and Medicare. Managed caseloads of up to 70 members simultaneously on a complex computer network utilizing Windows, Case Trakker, Outlook, QMACS, QNXT, Predictive Pathway Modeling, and various standard office programs. Primary interaction was with members, their family, physicians, nurses, and support staff. Heavy emphasis was placed on patient education and reinforcement in regard to CAD, CHF, and COPD while simultaneously diagnosed with serious mental illnesses. Was also responsible for training and serving as a resource person for new staff and leading a team of care coordinators and member services representatives throughout their daily assignments and objectives.
