The service will help meet growing national needs and address the important issues relating to inappropriate extended placements in secure, or other, inpatient environments. In particular, the service recognises the complex nature of challenging behaviours, the need for 24 hour care, support and the requirement for an experienced clinical team to manage risk professionally in the community. We offer highly individualised and tailored care packages for service users who do not need the physical security of an inpatient facility but do require support caring for themselves in the community, thus reducing the likelihood of relapse. Gray Healthcare has a bespoke “search and select” property team who have a National Contact Framework of Social Landlords, enabling us to secure tenancies for most service users, including those with complex needs, challenging behaviours and Community Treatment Orders. The comprehensive clinical package includes nursing, psychological support and occupational therapy within a framework of bespoke risk management strategies directed by an NHS consultant psychiatrist (who may be the service users Responsible Clinician in the community). Each patient’s needs are individually assessed and a full community based care plan is put in place which, typically would involve a minimum 4 and up to 24 hours per day 1:1 time with allocated staff. As service users progress, the time input can be varied to meet new horizons or reduced as the service user moves towards greater independence. Our unique dual registration with CQC ensures that we are able to offer a comprehensive “in patient” approach to care in the service users own home. -
