Creating and updating personalized care plans for patients.• Collaborating with patients' families, friends and social supports in developing treatment plans.• Identifying and recruiting high-quality healthcare providers.• Interacting with healthcare providers to facilitate patients' treatments.• Ensuring patients’ treatment requirements are met.• Suggesting alternative treatment plans when patients' services requests do not meet medical necessity criteria.• Instructing and educating patients on procedures, healthcare provider instructions and referrals.• Linking patients to social services programs and entitlements such as transportation assistance and translation services.• Conducting regular follow-ups with patients to evaluate progress, promote continuity of care and ensure improved health outcomes.• Maintaining records of case management activities.

• Compiling, Organizing and Analyzing Data• Counseling, Job Placement, Job Coaching, or Public Vocational Rehabilitation Program.• Proficient in Planning, Organizing and Coordinating Work Assignments• Completes assigned tasks as written notification thru TTD (things to do) with 97-100% accuracy and exceeds a certain time frame.• Support VRC in service delivery and accurate case management in meeting federal and state mandates as documented by monthly RIMS reports. Assist VRC with technical and clerical duties.• Perform professional and courteous customer service consistently for both internal and external customers such as VR staff, coworkers, supervisors, vendors, employers, community partners and consumers.• Provides assistance to VR internal and external stakeholders: Examples include, but are not limited to: Outreach activities, DVR presentations, Vendor recruitment and assistance, Identify comparable benefits, job placement assistance and follow-up, career workshops, VR orientation.• Processed financial functions according to approved policy/procedures measured by TTDs and/or Monthly Performance Reports (VRT Action Plans). • Work closely with Administrator in making appointments, scheduling, and filing/organizing medical documents.
