Mental Health Counselor
CurrentCollaborates with clients of this residential treatment program for people 55 years and older to develop critical social skills and coping strategies to manage their mental health challenges. • Conducts client intake interviews and Community Functioning Evaluations (CFE)• Enters daily client progress notes, assessments, treatment plans and closing summary notes in Epic (medical records)• Monitors and records client medications and places orders for prescription refills• Connects clients with healthcare providers and assists clients in scheduling and attending appointments• Prompts clients to follow a consistent routine in taking medications, participating in daily activities and practicing good sleep hygiene• Responds to client’s medical and psychiatric emergencies• Supports client’s effort to implement their treatment plan through interventions and monthly reviews• Leads groups and participates in outings and other planned recreational activities • Relays critical information with other staff during shift change and weekly staff meetings• Complies with all applicable federal, state and local governmental laws and regulations, as well as all agency policies and procedures, including the Code of Ethicsz
