Current• Provide supports for residents with specific attention to substance abuse and harm reduction • Schedule, conduct, and document individual coaching sessions. • Maintain client records to include documentation of progress. Ensuring that necessary forms, reports and progress notes are properly completed and contain informative descriptions of the services provided and the residents’ responses to the services. • Complete discharge procedure including documentation, aftercare planning, referrals, and check-out. • Assist client to identify values, preferred future, and barriers to getting there. • Maintain availability for clients through an open-door policy• Assist client in developing Personal Recovery Action Plan • Prepares psychosocial assessments and notations in the client's chart. • Ensure implementation of behavior management to director, interpret and explain resident behavior to director in a way that fosters understanding, facilitates treatment and respect for residents’ rights. • Participate in community planning, development and implementation of social care plans, resident assessments and discharge planning to meet the needs of the facility, the residents and their families. • Develop, design, and maintain agency’s computer system to maintain client records to include documentation of progress. • Disperse, monitor, regulate, and order medication procedures.
