To monitor and guide clients in their daily house setting such as meal, hygiene and medications. Provide resources if needed. Ability to understand and follow safety procedures. Ability to assess and provide for the day-to-day needs of mentally, emotionally and/or developmentally disabled patients. Ability to safely lift, and physically manipulate patients if needed. Skill in preparing and maintaining patient records.

Room and triage patients, vitals, update medication lists, review and update care gap in chart, pend labs, tests and imaging for PCP, POC test, draw up and administer injections, EKGs, draw blood if needed. Also work phone triage to gather information on incoming calls and route to provider or clarify patient information. Call in medication and refill medications according to office guidelines with exception of narcotics. Within Epic system response to Mychart messages, callbacks, scan documents in patient chart, Check in, check out, schedule and reschedule appoints for patients when needed.

Ability to understand and follow safety procedures. Ability to assess and provide for the day-to-day needs of mentally, emotionally and clients in drug recovery. Ability to safely escort clients in their room or out the building with assistance from security. Data input in computers, and routine shift room checks. Skill in preparing and maintaining patient records.

Assisted with patient care with triage vitals, updated Med chart paper and electronic. Preformed EKGs, POC tests, injections and draw blood. Assist medical secretary in calling in refills and chart updates.

Providing basic personal care, such as bathing, dressing, and grooming. Performing light housekeeping duties. Replacing sterile dressings and monitoring wound healing. Assisting the patient in an out of bed and/or transferring to or from a wheelchair.
