Serving as a Senior Quality Improvement Specialist from 2021, I led the recruitment, training, and guidance of healthcare facilities in implementing the PRiSM digital health remote patient monitoring platform, leveraging biometric data from API-enabled devices and patient-reported symptom tracking. I developed instructional materials to facilitate the seamless integration of the PRiSM platform with individual clinic workflows and diligently managed both project deliverables and client expectations. Concurrently, from 2017, I assumed the role of a PCMH Practice Facilitator, where I assisted in the management, development, implementation, and evaluation of a variety of public health initiatives through health information technology and quality improvement projects. Serving as a trusted advisor, I engaged with a diverse array of healthcare entities, employing organizational development and project management methodologies to aid clients in reaching both incremental and transformative improvement goals. As a Project Coordinator for Project LAUNCH, I managed a child wellness screening program in two of the largest pediatric clinics in Rhode Island, serving economically disadvantaged and minority populations.
