Serving as a Senior Quality Improvement Specialist from 2021, I led the recruitment, training, and guidance of healthcare facilities in implementing the PRiSM digital health remote patient monitoring platform, leveraging biometric data from API-enabled devices and patient-reported symptom tracking. I developed instructional materials to facilitate the seamless integration of the PRiSM platform with individual clinic workflows and diligently managed both project deliverables and client expectations. Concurrently, from 2017, I assumed the role of a PCMH Practice Facilitator, where I assisted in the management, development, implementation, and evaluation of a variety of public health initiatives through health information technology and quality improvement projects. Serving as a trusted advisor, I engaged with a diverse array of healthcare entities, employing organizational development and project management methodologies to aid clients in reaching both incremental and transformative improvement goals. Before these roles, between 2016 and 2021, I was the Regional Lead for Beneficiary Education, overseeing regional and state teams under a CMS contract to enhance diabetes self-management education. I recruited, trained, and managed a network of certified leaders, monitored and elevated performance on CMS deliverable targets, and served as a liaison to the Patient and Family Advisory Council (PFAC).
