This involved project work and pathway redesign to deliver higher quality more integrated services as well as representation of the CCG externally on relevant topics. My portfolio also included contract management and developing the CCG Constitution and interpractice agreement. KEY ACHIEVEMENTSI developed a new model to improve the quality of primary care in care homes and to reduce Care Home related admissions to hospital. This model aligns one practice to one home to provide a proactive approach to care homes. I successfully gained CCG and practice sign up through presentation at the practice forum, a robust business case at the CCG Board, use of the LMC’s support and continued clinical involvement in the project. I led implementation and evaluation of a pilot of this service which demonstrated improved care, reduced acute admissions and financial savings. The pilot is soon to be rolled out. Show less

I successfully gained approval from all three PCTs with new funding from two, produced implementation plans and led the start of this to maintain consistency across all PCTs.• I encouraged partnership working through implementation of a dementia strategic group incorporating health and social care commissioners across the whole county. I introduced and managed performance management measures through this group to ensure all 5 partner organisations were delivering on the dementia strategy and to facilitate reporting to the SHA. • I led partners from health and social care provider organisations across Nottinghamshire and Nottingham in taking a joined up approach to planning of dementia training. For the acute organisations this approach was to support them in delivery of their dementia CQUIN targets. This resulted in production of a competency framework for dementia training with full sign up to deliver from all the organisations, a joint dementia workforce plan and development of a joint training course.
