Senior Behavioral Health Linkage Worker (Bhlw)

- Responsible for accessing behavioral health services and providing a warm handoff for patients with behavioral health conditions, who have been identified and referred by the primary care providers through the Annual Behavioral Health Survey (ABHS) or from the doctor’s visit- Utilize surveys and behavioral pattern analysis to perform depression and anxiety screening for 800+ patients, raising the total depression screening rate of patients past the organization goal of 80%- Assist patients to schedule appointments with NEMS behavioral health providers, including ACSWs, LCSWs, and other professionals on the team- Assists patients to access behavioral health services from County behavioral health clinics, community-based behavioral health clinics, and substance use treatment programs- Follows up with patients who did not show up for their initial behavioral health appointments to encourage compliance with the referral orders from the primary care providers- Designing and executing the Annual Behavioral Health Survey on all patients - Provides case management activities as requested by ACSWs, LCSWs, and other professionals on the BH Team- Documents referrals and case management activities in the patient’s electronic health records, and submits billing as instructed- Provides outreach to patients at high risk of co-occurring physical and behavioral health disorders- Lead the department’s continuous technical development of the multiplatform ABHS- Updates the Behavioral Health Referral Resource Database regularly- With a team, generate frameworks and insights that guide Social Workers’ decisions on treatments- Present analysis and strategic insight on improving treatments based off of post-session surveys - Work with ACSWs transitioning out of NEMS to reschedule + reassign patient load- Prepare and conduct training for new hires with an emphasis on empathetic patient experience
