Support person in community access e.g., involve to organize and participate in activities such as picnic, day trip, movies, dining out with family and friends, birthday/farewell party, community retreat, key connection dance, etc. Support person to develop and work towards progressive achievement of personal goals around social skills, communication, household cleanliness, cooking/baking, social connections, etc. Prepare menus and meals according to dietary guidelines, needs and preferences. Administer medication. Accompany people to various medical appointments such as doctor, dentist and other specialists. Document medications, daily activities, goal achievements, behaviors, finances and incidents. Provide support in their personal spiritual journey e.g., to participate in house prayer, church activities, bible study and community weekly prayer. Take responsibility and follow through on commitments; listen actively and communicate effectively; able to adapt to changing needs; collaborate and support team leader and team members; understand the impact behavior has on others: contribute to positive team dynamics. Responsible for monthly car and house maintenance checklist, grocery list and shopping.
