Working on inpatient hospital services, including internal medicine, vascular surgery, cardiothoracic surgery, and trauma. Provide medical social services to patient and families, formulate and implement patient’s discharge plans, and provide assessments as needed. In this role I am a case manager and discharge planner. When hospitalized patients become medically stable to discharge, I helped coordinate that discharge to a safe environment. I am responsible for referring patients to physical rehabs, nursing facilities, home health services, home oxygen services, durable medical equipment supplies, and so on to ensure a safe discharge. Also addressed any relevant psychosocial needs that arose such as family issues, legal guardianship, advanced directives, community resources, etc.

Research assistant for a RN PhD conducting a study regarding lung cancer and stigma. As research assistant I primarily collected data for the study. I collected this data at UK Markey Cancer Center where I administered a questionnaire to patientswith lung cancer. I asked patients the questions and recorded their responses. I then entered this information into a databasefor results to be aggregated. I was also responsible for the safe storage of this data and keeping in compliance with HIPAA.

Student under CSW social worker in the acute care hospital case management department. Experience in case management, patient charting, resource management, discharge planning, facility placements, patient consults, and interdisciplinary teamwork. I assisted with implementation of PsychoSocial Distress screening program to be in compliance with Commission on Cancer standards. This entailed developing a system to administer the PsychoSocial Distress Thermometer (DT) scale to patient's identified with cancer. Hospital staff was able to identify those patients with cancer throughout entirety of the hospital and would send a list to me and I would administer the DT which I would then put in patient’s chart. When I left this position I had talked extensively with my supervisor so that she could resume the process.

I was a social work practicum student at Christian Care Communities which is an adult day care for people with Alzheimer's, and I also spent time at the Best Friends program which is also an adult day care center run out of a church and affiliated with CCC.
