Authentic Life Care Services offers fee-for-service advocacy and case management services to aging, disabled and incapacitated clients. We serve as "chosen family" for clients who may not have capable, local or trustworthy family to care for them in times of decline of personal capability or when temporarily or permanently incapacitated.
Each client's needs and our services to meet them are individualized. In supporting our client's preferences and / or meeting court-appointed responsibilities to care for and protect our clients, we may:
• Coordinate medical, financial, legal, and social care services as Agent authorized by individual Power of Attorney and/or as Guardian appointed by court decree.
• Apply for and coordinate delivery of entitlement services for client under public benefits programs for disabled and/or incapacitated persons.
• Provide daily money management, real estate property management, and personal property liquidation services as necessary to support the safety, health and welfare of clients.
• Manage and conserve client assets to protect clients from financial exploitation. Serve as Social Security Representative Payee and/or as Veterans Administration Fiduciary when appointed for these duties.
• Select, hire and advocate for clients in overseeing delivery of services by providers such as healthcare professionals, long-term care facilities, home care services, hospice agencies, real estate professionals, landscape services and property managers, design and construction services, investment and banking services, and social services agencies.
• Provide informed consent for medical procedures and orders, including end of life decision-making based on clients' documented advance directives and/or communicated preferences and wishes for their personal definition of quality of life.
