We are known as the founders of The Hogeweyk®. It was our first attempt at a complete de-institutionalisation of the traditional nursing home model. Not an institution, but a place where you can live your life as ordinary as possible. It sounds easy, but doing this sort of thing in healthcare is pretty complicated. How do you go from intention to doing? Enabling people with dementia to live longer at home also requires a radically different view in which the focus is not on the disease but on the individual with all his individuality and his own interpretation of life. Be@Home is an integral care concept for people with dementia at home based on a holistic and social approach. How to implement and secure this approach effectively and with great qualitative outcomes?
