Careteam enables clinicians to create care plans for patients with complex and chronic health issues that include instructions and recommended resources. The patient or their main caregiver can then share the care plans with who they see as their circle of care and Careteam enables the activation of the care plan into a to do action plan. Health organisations can monitor whether the plan is being actioned and proactively intervene to prevent deteriorations.
