With the ongoing development with an interconnected world information is now universally assessable. Communication has also been enhanced enabling real time collaboration between health care providers as well as providing secure electronic access of medical records by patients as well as patients contributing to and sharing their medical files. Collaborative Health Service is based on the premises of a shared partnership between health care providers and patients. It is our aim to empower patients and their families to take an active role in their health care. We will provide support and guidance to help you maximise healthy life choices to meet your personal wellness goals. We will also inform you of current health controversies and provide and lead you to key information sources to enable you to access the evidence for yourselves. We aim to do this with cutting edge technology to enable real time communication and collaboration. We will not shy away from addressing difficult life choices that come up in the various stages in the life cycle. With advancements in medicine and an aging population there is an ever increasing burden of chronic disease being managed by multiple providers and agencies. It is our aim to guide you through this maze and both inform you and facilitate access to a range of services. We address ourselves to the broader Australian community to help with equitable access to services and information. We are involved in multiple settings from the classic family unit to those needing residential and psychological support. Through these aims we hope to contribute and inform the ongoing discussion of the content and direction of our health service. We are here to advocate, enhance and challenge beliefs and service provision.
