Dementia Support has transformed pre and post diagnostic dementia support in West Sussex with the Sage House Model.
Sage House offers high-quality, fully integrated person-centered care & support for people living with dementia and their carers with a range of services under one roof including advice, guidance, the NHS dementia assessment service, legal advice, activities, respite day care, and wellbeing services care.
The charity’s vision is “a society where dementia is wholly understood and accepted, enabling people living with dementia to be fully supported throughout the whole of their journey.”
Since its launch in 2014, Dementia Support's Sage House has proven agile and adaptable, by moulding to the needs of the community and its customers.
With 944,000 people currently living with dementia in the UK, and the number predicted to reach 1.6 million by 2050, it has the potential to save the government billions in health and social care costs over the coming years if delivered nationwide. It also demonstrates that there is a better way to support people living with dementia and their care partners.
The Sage House Model is scalable and flexible and is now available for community adoption across the UK.
