The Eastern Sector Development Team (ESDT) support the transition to the Support at Home and implementation of inclusive aged care services, built on the principles of equity and person-centred practice. We deliver this through the promotion of responsive diversity, wellness and reablement approaches that focus on the strengths and abilities of people, their preferences, needs and aspirations. We are funded to work with Commonwealth Home Support Programme (CHSP) services in the eastern metropolitan region of Melbourne.
