Health Conscious is a digital health education and improvement platform that helps people with chronic health conditions, such as diabetes and high blood pressure, fully understand how they can remove the lifestyle risk factors associated with their conditions and take practical steps towards a healthier mind and body. The platform, available on both website and mobile app, consists of three online courses - High Blood Pressure, Diabetes, and General Health. Each course consists of a weekly education video, group coaching calls, action steps, community Q&A forum, and other interactive features. We also incorporate CBT techniques and stress management techniques as a key focus for health improvement, in addition to the traditional emphasis on diet and exercise, emphasizing a mind-body awareness. Health Conscious is supported by medical authorities from specialties ranging from Internal Medicine, Cardiology, Neurology, Nephrology, and Pulmonology, as well as preventive health researchers, psychologists, dieticians, and wellness coaches, incorporating the results from thousands of clinical studies. These various perspectives ensured that we help users gain medical understanding and then the action steps to implement that knowledge. Our mission is to decrease the chronic health epidemic, specifically through increasing health literacy and addressing social isolation.
