Harpenden Health PCN is a Primary Care Network (PCN) set up in 2019 and compromising 3 high achieving GP practices in Harpenden, Hertfordshire.
Together we care for more than 45,000 patients and are excited about what the future holds for Primary Care in Harpenden.
We aim to provide a patient friendly quality service; we enjoy multidisciplinary teamworking and the opportunities that collaborative working in a PCN provides. As a PCN we have been able to further develop our team which now comprises of not just GPs and Practice Nurses / ANPs but has also expanded to include Care Coordinators, Clinical Pharmacists, Dietitians, First Contact Practitioners, Paramedics, Health & Wellbeing Coaches, Mental Health Practitioners, Physician Associates & Social Prescribing Link Workers.
By working together as a network and with the additional team roles, the 3 practices are able to offer increased services at scale.
