The Minto-Mapleton Family Health Team (MMFHT) consists of a group of health professionals working together with 9 primary care physicians and 6 nurse practitioners who have a combined patient roster of approximately 15,000 patients. We have locations in Drayton, Palmerston, Clifford and Harriston and service the Minto and Mapleton communities.
Using an interdisciplinary team approach to health promotion, prevention and chronic disease management, the MMFHT provides services promoting healthy lifestyle to improve your mental and physical well being. In addition to our nine physicians, our team includes nurse practitioners, registered nurses, social workers, registered dietitians, kinesiologists, a physician assistant, a part-time pharmacist, an outreach worker, and administrative support.
If you are a patient of a provider located in the Drayton, Palmerston, Harriston or Clifford areas, you're also a member of the MMFHT. This means you are able to access a range of programs and services (free of charge) that are provided by the employees of the MMFHT. Currently, we are offering care and support to their patients by scheduled in-person, phone, or video visits.
