Founded in 2012, Prime Horizon leading the charge in providing day services and supportive home care services to adults and children with developmental disabilities, physical disabilities, and the elderly. Owner’s Shawn and Melanie identified a shift in the health care industry early on and felt compelled to develop these types of specialized services and offer them to a broad range of clients in both a safe and nurturing environment.
At Prime Horizon, we focus on delivering an impactful and life-changing experience that empowers our clients by encouraging a high level of independence and self-directed care.
Over the years, we have seen a tremendous level of success in the programs that we offer and we have continued to develop entirely new services that aim to only further the support of our clients.
Some of these specialized programs include:
✔ Our highly individualized custom home care program which provides non-medical home care to individuals right from inside their home (Ozaukee County area only)
✔ Our Supported Apartment Program which enables adults with disabilities the opportunity to live independently with the support of our seasoned Prime Horizon staff
✔ Our Adult Day Programs offered in a safe and nurturing environment.
We are proud to be serving in this space for almost a decade and we continue to see strong demand for our specialized services!
With the help of Prime Horizon, individuals of all ages with an array of disabilities have been able to transition from higher-cost long term care settings to their own apartment.
