PSYCHeANALYTICS offers a pragmatic triage tool for universal Behavioral Health screening to reduce healthcare disparities and give Primary Care Clinicians useful knowledge regarding their patients’ behaviors and social conditions that determine 80% to 90% of health outcomes.
Patients are contacted on behalf of their provider’s office via text messages and given a unique, personal link to complete the screener on their cell phones at home. Triage evaluations are then sent to the patient’s chart immediately, enabling proactive care coordination and opening the door to whole-patient care.
