About The Care Partner Project
The Care Partner Project, 501c3, based in Chicago, was founded to help solve for twin "disconnects" in the US health care system: 1. Approximately 60 million people in the US coordinate medical care for a family member with a serious health condition, yet, nothing in life prepares anyone for this important role. 2. Medical care providers are increasingly stretched. They don't have the time to coach patients and families, let alone coordinate care. The Care Partner Project equips patients, families and providers with information and resources to bridge these disconnects. We set them up as Care Partners to support the patients in their care. How? With resources that inform, engage and activate in simple terms and friendly graphics. 1. Free, downloadable, “grab and go” checklists that coach patients’ families in what to do, say and watch for to support safe and sound care during hospital stays, ER care, childbirth, doctor appointments, more. Usable on IPads, smart phones, in print. 4. A PFE program for hospitals our checklists guides as their own to share with patients and families on their own websites, patient portals, and patient resources centers, in print or e-format. 2. Highly engaging community presentations, delivered at libraries, Rotary Clubs, senior communities, (more) by professional patient advocates, currently covering 52 US cities.
