We are a faith-based, non-profit home health and hospice agency in Minnesota. Triniti Home Health & Hospice is jointly owned by Guardian Angels Senior Services of Elk River and Cassia, an Augustana/Elim affiliation. From the comforting care we provide in hospice, to the restorative focus we have in home health care, our synergistic teams set us apart from other agencies. Triniti Home Health & Hospice employs a community of health care professionals consisting of nurses, social workers, therapists, chaplains, music therapists, massage therapists, volunteers, and more.
