I aided in undertaking Clinical Reviews for clients from Palm Cove to Cardwell in Far North Queensland. I assessed some clients remotely. I treated chronic wounds and liaised with GPs about management strategies between the clinic and in the community for an overall better holistic approach. I aided in reviewing clients, undertaking reassessment reports for ACAT, and assisted in clinical oversight of their Care Plan and clinical-specific concerns. I supported the Case Manager with their clients and aided in fulfilling Palliative Care shifts in the community for clients who required increased support in the community. I assisted with questions supporting Advanced Care Planning, and trained staff internally about deteriorating clients and what to look out for. I liaised with allied health staff for advice about what the best approach for certain situations could entail for a well rounded, and clinically supported plan for the client to determine whether it is right for them. I built rapport in short periods of time, and trained staff from Far North Queensland to the GoldCoast regions with internal mandatory training, with best practice guidelines. I supported senior management with the development of the Palliative Care procedural pathway and policy for the company and started to introduce Palliative Care Packs in Cairns and Brisbane South regions, for informative care solutions to aid in keeping the client at home if they so wished.
