• Manage General Inpatient Admissions and Hospitalized Team Patients• Provide Case Management of Patient Care, including coordination of symptom management with ordering physician and hospital case management• Assures appropriateness of orders/treatment and effective symptom control while managing length of stay and patient inpatient throughput.• Use established guidelines to evaluate referred patients for Hospice appropriateness, through physical assessment, hospital chart review including History and Physical, physician notes, diagnostic testing, lab tests and other relevant information. • Acts as a liaison between referring hospital/agency and Hospice prior to discharge; assures communication and collaboration between all involved agencies, facilities, physicians and staff. • Provides Hospice Information to patients/families, prepares them for admission, initiates admission process, including principles of effective symptom management, and completes admission paperwork including verbal and written explanation of Patient Rights and Responsibilities and other consents required by regulation. • Notifies ordering physician and hospital case management that patient is hospitalized for management of symptoms related to Hospice diagnosis that patient care is being covered under the election of the hospice benefit. Notifies patient and patient physician when care/testing/treatment being ordered is outside scope of Hospice plan of care and impact of such decision on their Hospice benefit. • Performs admissions procedures as a back up to admission team. • Provides discharge planning for patients within the hospital setting and initiates prescriptions, medical equipment, staff visits, and warranted crisis care. • Admits hospital referrals as General Inpatient at the agency established conversion rate • Leads Medical Communication Education Activities• Hospice and Palliative Nurses Association Member 2013
