Work with SASH participants to provide the guidance, health coaching, and education necessary to maintain safety, prevent unnecessary and costly hospitalizations, and facilitate their continued active participation in community. Work in collaboration with primary care providers, home health agencies, local Area Agencies on Aging, mental health agencies, and area hospitals and rehab facilities to provide a wrap-around of person-centered services. Responsible for handling and overseeing the day-to-day operations of an 80 unit affordable housing site for seniors and people with disabilities.
