KW4 Community Ward (CWT), is a multi-disciplinary team providing in-home outreach services and primary care as an extension to support primary care providers with patients that are medically and/or socially complex, in a dignified and holistic patient-driven approach.We are an innovative multi-disciplinary team that do in-home visits, specializing through an SDOH approach, discovering needs that would not have been identified through traditional health care strategizes​Through listening and relationship building, I aim to understand and work with the patient holistically by meeting the patient's goals first, directly helping patients get to a stable state.​As an OW, I closely follow and coordinate patients' needs until the goals of the patients themselves and/or referrers' goals are met. The team diligently makes sure supports are also in place. ​This is done by regularly reviewing caseloads, rotating and sharing MRP roles, all driven by patients' immediate needs.Finally, I am part of a team that is aware and conscious of biases and stigma associated with the patient population being served.
