One thing I do to support this process and make it sustainable is to do social prescribing and to be effective at this I need to keep my finger on the local community pulse. I need to be aware of the the services, community groups, local assets, organisations, initiatives and key people around me so I can refer, signpost, advise and inform my client's in the best way so as to serve them and the community.
