I’ve been a clinical preceptor for six PharmD students since October 2021 for the Experiential Education course PHARM 556 Advanced Pharmacy Practice Experience Part 3 Selective in Direct Patient Care, through University of Alberta's Faculty of Pharmacy and Pharmaceutical Sciences. This 8-week (320 hour) structured practical learning experience will allow students to apply and integrate knowledge and skills in a patient care setting. Students will develop practical knowledge necessary for the professional role of pharmacists as care providers, communicators, scholars, educators, collaborators, advocates, leaders and managers. Students are expected to step into the role of a pharmacist under the guidance of their pharmacist preceptors. Precepting students provides me an opportunity to nurture and promote the profession of pharmacy, develop personally, and influence the maturation and education of our future colleagues.

Within our PCNs Senior Services programs, I developed and optimized successful collaboration between our patients, primary care physicians, allied health care team, and specialist consultants. I generated opportunities for geriatric-focused Continuing Medical Education and Collaborative Care Education, facilitating skill enhancement of our physician members and their allied health Collaborative Care Teams. I report directly to the Director of Clinical Services, and indirectly to the Executive Director, while working alongside the Physician Lead for Senior Services.

Our PCN's primary care physicians refer to our program their physiologically-aged patients who could benefit from additional diagnostic evaluation, medical management, patient education, family support, and system navigation. With each patient, I provided medication therapy optimization, in partnership with our team that includes the patient, their caregivers, our nurse consultants, and our phyicians.

In the medical offices of our member physicians, I was part of the collaborative care team who sees patients in the primary care setting. Patients were referred to my evaluation by their physician, related to poor results of existing medication therapy. Using the referral information, patient chart, medical history, patient assessment, laboratory testing, medication review, and patient preferences, I created new medication therapy management strategies to improve safety, tolerance, and effectiveness of treatment. The patient, their physician and myself teamed to initiate and monitor the new treatment for outcomes.

Coordinated patient-centred pharmacy services. Provided patients a combination of primary and secondary care via a team of 10-15 individuals, including clinical pharmacists, pharmacy technicians, pharmacy student interns, pharmacy assistants, and internationally-trained pharmacist interns. Created growth in pharmacy services to meet more patient's needs, which consolidated demand, achieving economies in provision of this care. Fostered an appetite for success in patient-centred care, by developing our team-member's competencies to match our increasing scope of practice. Developed a health-care destination for patients, in a diverse community with diverse needs.

Fostered a healthy community by providing expanded patient-centred pharmacy services. Delivered primary care and secondary care to a community of 7000 residents. Developed a medication assistance program for the Parkland Senior Citizen Lodge. Provided community and school education on health-care. Precepted pharmacist interns on community-based pharmacy services. Expanded the health care community in Edson. Provided unique services and products to our area via a team of 30 employees.

Providing patient-centred medication therapy services. Primary care services include triaging patient based upon their complaints, routing them to the appropriate care provider, and where appropriate, offering self-treatment advice and self-monitoring education. Secondary care services include collaborating with patients and their prescriber to provide the appropriate and tailored prescription medication therapy. I ensure my patients are receiving the most appropriate medication, by collecting their diagnosis, their medical history, my assessment, the available therapeutic choices, and their treatment preference. When supplying medication treatment, I provide patient teaching and training on their diagnosis, the need of treatment, goals of medication therapy, instruction on use to achieve these outcomes, expected benefits and their timing, unexpected effects or rare risks, advice on how to prevent these, or manage them if experienced, under what circumstances or when to present themselves for re-evaluation, duration of treatment, and the risks of non-adherence.
