Patients were referred to my evaluation by their physician, related to poor results of existing medication therapy. Using the referral information, patient chart, medical history, patient assessment, laboratory testing, medication review, and patient preferences, I created new medication therapy management strategies to improve safety, tolerance, and effectiveness of treatment.
