Participates with the physician, referral sources, director and professional staff in decisions regarding patient eligibility and suitability for the hospice program and coordinates hospice care services from admission through bereavement.Key Accomplishments• Collaborate with Hospice Interdisciplinary team leaders to promote the provision of quality Hospice Care in all settings; direct supervisory oversight of the Hospice disciplinary teams.• Provide on-going staffing and education for staff, volunteers, and relevant departmental employees.• Partner with outside vendors to provide continuity of care if and when additional staffing is necessary to insure optimum patient care; assist in development and implementation of policies and procedures• Interview and recommend for hire new employees; ensure all state and federal regulatory requirements are met• Counsel, resource and mentor staff; develop and monitor action plans for staff.• Responsible for annual staff “performance review” process.• Work with Quality Management to insure the highest quality of care and compliance with JACHO, Medicare and federal and local regulations and requirements are met.• Work jointly with Program Director on current and future budget projections and practical implementation.

Key Accomplishments• Managed and directed the in-house clinic for 300 independent residents as well as 20 assisted living residents. • Oversight of 2 MD’s, 3 nurses, 2 medical support positions and 6 CNA’s.• Managed and cared for emergencies ranging from falls resulting in fractures to skin tares, chronic conditions such as dementia, diabetes, congestive heart failure, COPD, Parkinson’s and range of other chronic illnesses.• Facilitated the use of hospice services for residents that have terminal conditions.• Created a newcomers group to ease resident transition from living in a private resident to a community setting.• At the invitation of management, I developed and deliver a lecture series on living happy and healthful lives.• Strategized and assisted with the implementation of new assisted living software and risk management software.

Key Accomplishments• Staffed all admissions for all company site; managed a team of 40 nurses providing twenty-four-hour care for all (three) Hospice By The Bay sites in San Francisco, Marin and Sonoma Counties• Set up staffing system that simplified process and is accessible via home computer• Educate after hours staff on use of Sun Coast (hospice software) application MDS system• Responsible for department budget; saved $70K last quarter by cutting back over-time and streamlining reporting system.• Advocated to replace a local after-hours answering service that was not meeting our patient’s immediate needs, to the use of a new “call center” that is staffed by RN’s and will provide a clinical response from the first call. This will provide a projected cost savings of $150-200K annually• Assisted with certification process for CHAPS, Community and Hospice Accreditation Program
