Facilitate group interactive lessons, lectures, and discussions for the Hand Up program participants; helping them understand the course information, using different teaching styles to reinforce essential information as needed.Meet all standards of the Hand Up programs while assisting and guiding students throughout the program with effective case management practices.Able to interact with participants who are part of the prison re-entry, homeless, and ALICE (working poor) populations; and those individuals who may be dealing with active substance use, and/or untreated emotional or physical health needs.Working in intense, emotionally charged situations as the need arises, as well as having the ability to respond safely and quickly in an emergency situation.Manage documentation and complete data entry with solid computer skills, utilizing programs such as MS Word and Excel and knowledge of, or the ability to learn the Apricot, the case management system used by The Christian Outreach Center.Displaying exceptional customer service, with a Christ-like attitude of acceptance, assessment of client’s needs are completed, with proven experience in community engagement and the establishment of good working relationships with both internal and external program staff, appropriate referrals can be made. Key areas of knowledge include local resources available for rapid rehousing and issues related to homelessness.Excellent organizational skills are necessary to ensure proper documentation while performing an array of difficult tasks simultaneously is necessary.

Manage care for approximately 40 clients who participate in the following programs:Medicaid Waivers including SIL, NOW, ROW, SW, PCS, PAS, CC/EPSDTPrivate Care including long-term care insurance plansVA Care through the Department of Veterans AffairsEnsure person-centered care plans are carried out by Direct Support Workers (DSW) and that clients' health and well-being are maintainedSupervise, train, evaluate, and provide direction to DSW in regards to all aspects of client careCommunicate frequently with clients, families, and other affiliates to answer questions, resolve issues, and ensure satisfaction with services.Travel to clients' homes for meetings and visits quarterly, annually, and as needed.Properly document all client communications, care plan or medical changes, visit notes, or incidents.Comply with all program guidelines and meet requirements established by LDH, Medicaid, and HCL.Promote HCL in the local community and build relationships with external stakeholdersParticipate in after hours on-call rotation and serve as backup staff working in-home with clients as needed

Create and implement staffing plan for shelter to ensure consistent, safe, and reliable staffing of shelter on a 24 hour basis.Hire, train, and supervise all shelter staff, including but not limited to Full-Time Residential Specialists, Part time Staff, and Maintenance WorkersEnsure accurate understanding and implementation by all shelter staff of policies and procedures through methods such as group meetings, individual meetings, policy manuals, checklists, documentation of non-compliance with policies and procedures, warning notices, and adverse employment actionsMaintain inventory of supplies for shelter, including ensuring that donations and supplies are used in the programs and no theft occurs, that needs for donations are reported to development, and that supplies are purchased when needed.

Develop recreational, physical education, and cultural programs for school age groups from 4th through 12th grade.Demonstrate and instruct participants in activities, such as active sports, group dances and games, arts, crafts, and dramatics.Organize current-events discussion groups, conducts consumer problem surveys, and performs similar activities to stimulate interest in civic responsibility and promote community awareness.Promote group work concept of enabling members to develop their own program activities through encouragement and leadership of membership discussions.Consult with other community resources regarding specific individuals, and makes referral when indicated.Keep records of all children enrolled in the after school program.Recruit, train, and supervise paid staff and volunteers
