Pac (Project Aids Care) Medical Case Manager
Current• Coordinate comprehensive services for caseload of 75 AIDS patients receiving Social Security disability benefits• Supervise Medicaid eligibility for program participants through Cares Unit and Florida Department of Children and Families• Conduct Needs Assessment and assess Level of Need qualifications to assign applicable services such as: Homemaker, Restorative Massage, Home Delivered Meals, Education and Support, and Specialized Medical Equipment And Supplies• Draft Plans of Care and Service Authorizations to ensure continuity of treatment • Manage therapeutic interventions and generate referrals to agencies and professional providers • Encourage self-advocacy by assisting clients with oral and written communication• Motivate patient proactivity to enhance wellness and optimize social and vocational opportunities• Research Managed Medical Assistance (MMA) plans to determine patient placements• Document and organize client and provider progress notes into electronic files for evaluation by Care Resource administrators and state auditors • Review laboratory results to promote patient/primary care physician communication• Engage in client Face to Face encounters and telephone consultations• Implement outreach interventions through hospital, rehabilitation, and home visit follow-ups • Bill patient services through Medicaid Portal and review financial statements• Attend in-service seminars and continuing education workshops on all facets of managed care
