David has a PhD in applied psychology and has worked internationally in the area of human reliability in systems for more than 30 years. He has published more than 130 papers, reports and a book on human factors in the process industry: 'Guidelines for Reducing Human Error in Process Safety' for the Center for Chemical Process Safety (USA). For the past 25 years he has been course Director for the Human Factors module of the University of Sheffield course in Process Safety and Loss Prevention. He has conducted human reliability analyses in a wide range of sectors, including petrochemicals, nuclear power generation, healthcare, aviation and rail transport. He acted as advisor to the public enquiry following the Clapham Junction Rail and Piper Alpha accidents, and as an expert witness for the Esso Longford plant gas explosion legal action in Australia. He has research interests in developing qualitative and quantitative tools to support human factors risk analyses in safety critical systems. He developed the SHERPA (Systematic Human Error Reduction and Prediction Approach) which has formed the basis for the UK regulatory approach to human factors risk assessments in the oil and gas sector. He also developed the Human Factors Risk Manager software tool to support these analyses.David is currently providing inputs into Safety Critical Task Analyses to oil and gas multinationals, and conducting research to develop improved communications within cardiac surgery teams using shared mental models. He is a Fellow of the Safety and Reliability Society, and a member of the Institute of Chemical Engineers, the Institute of Ergonomics and Human Factors, and the Human Factors and Ergonomics Society (USA).
