* Health coaching – guiding people living with diabetes to meet their health goals. *Development of the workforce through planned and ad hoc education. E.g. develop and provide weekly in-service education to ward nurses. E.g. write a weekly news email for all clinical staff. E.g. deliver education sessions to GP practices. E.g. in liaison with Nurse Managers established local ‘Diabetes Champion’ network and provided advanced diabetes clinical education to this group. *Provide leadership and coordination of diabetes clinical services within the Bathurst hospital and local community, including GP practices e.g. support medical and nursing staff in the care of inpatients, including discharge planning. E.g. supporting GP’s in the care of complex diabetes patients. e.g. develop strategies and systems to enable all PLWD had access to the service. E.g. acted as a resources for all clinical teams managing inpatients. *Planning services based upon the needs of the population and changes in the health system. E.g. assessing the number of women being diagnosed with GDM and providing sufficient staffing hours to cover needs. E.g. assessing the needs of PLWT1D who use CGM and insulin pump therapy and providing sufficient staffing hours to cover needs. *Developing and monitoring the clinical effectiveness of service and delivery systems. E.g. collaborating with other team members to review and develop care plans for PLWD being admitted on a frequency of more than 3 times per month. E.g. reviewing care plans with junior medical staff to ensure safe and effective planning has been undertaken. *Assisting the Local Health District (LHD) executive in developing policy that supports the changing needs of the population and fosters innovation whilst remaining consistent with the goals of the LHD strategic plan, and that of NSW Health.
