The TAP-in provides cross-cutting and tailored technical assistance to 47 jurisdictions (i.e., health departments) across the nation who are funded by the Ryan White HIV/AIDS Program (RWHAP) to design and implement locally tailored plans to improve access to treatment, foster retention in services, and achieve durable viral suppression for people living with HIV as part of the nation’s plan to Ending the HIV Epidemic.

•Develop, monitor, evaluate, and revise Georgia Ryan White Part B Clinical Quality Management Plan•Facilitate Clinical Quality Management Team performance measures subcommittee and state office Quality Management Improvement committee meetings•Provide technical assistance related to new policies and implementation for territories for clinical quality case management and Aids Drug Assistance Program (ADAP) with HIV/Aids Bureau (HAB) standards and implement quality improvement projects for 26 health district territories in Georgia •Develop and Update Clinical Case Management Performance Measures and related training materials within programmatic guidelines•Act as liaison to educate County Health Department staff on appropriate available patient support programs, explain financial assistance, and other funding options available to the patients •Conduct Statewide HIV Clinical Case Management Chart Reviews, performance indicators, and needs assessments for Ryan White Part B

•Decrease barriers to accessing health care by 15% at local health department•Increase medical adherence by 30% through client centered problem solving improving supportive services, incentives, and eliminating barriers to healthcare •Advocate and crisis intervention for individuals living with HIV and LGBT population•Assess the primary and immediate needs of people with HIV, coordinate referrals, and follow-up with critical core medical and support services to ensure people with HIV remain in medical care•Reactively address access to Antiretroviral medication and provide necessary education for clients to secure appropriate reimbursement and access•Interact with healthcare providers, to understand access and adherence issues and work toward resolution while maintaining communication with all cross-functional partners throughout interactions and follow through to resolution

•Facilitate collaboration meetings with internal and external work groups comprised of DOL employees, nonprofit administrators, and community leaders•Supervise work of assigned staff and performs delegated administrative and personnel management responsibilities such as interviewing , guidance, recommending appointments, and disciplinary action•Provide leadership in planning, developing, implementing and modifying programs and activities to meet requirements and objectives of the agencies strategic plan•Oversaw employment and/ housing program with 110% goal achievement •Placed 350 homeless veterans into competitive employment •Oversee contract management of Homeless Veterans Reintegration Project and SAMSHA funded Substance Abuse Program •Prepare reports, presentations, and supporting documents for Senior Leadership on program progress and identify potential federal funding opportunities to sustain organization services•Evaluate program data and complete quarterly program reports •Manage program annual budget of $300,00 to track line item expenditures of Federal Grant Projects•Participate in annual Commission on Accreditation of Rehabilitation Facilities (CARF)
