Executive Director Of Care Management
CurrentOversees and directs the operational effectiveness of inpatient and outpatient care management and clinical integration activities across the care continuum promoting seamless post-acute transitions between care settings and facilitating the appropriate use of healthcare resources. Encompasses leadership of care management, clinical resource management, utilization review, social work services and discharge planning. Spearheads the development and implementation of inpatient and outpatient care management strategies, fostering a patient-centric approach to healthcare delivery. Serves as an expert resource and mentor across the enterprise to physicians, clinical staff, administrative staff, internal/external stakeholders and regulatory agencies in all issues related to care management and continuum of care services.
