Clinical• Provide support to 140 dialysis patients. • Complete biopsychosocial assessments with new dialysis patients, develop plan of care, administer standardized screens for depression (PHQ-9), quality of life (KDQOL), and distress (Distress Thermometer). • Provide education on treatment modalities (in-center hemodialysis, home hemodialysis, peritoneal dialysis, and comfort care), transplant, and depression. Work with interdisciplinary team to provide patient education around fluid intake, renal diet restrictions, and kidney disease. • Provide brief, solution-focused therapy to patients, as needed. Identify and work to resolve potential barriers such as lack of insurance, transportation, and education.Administrative • Complete site standard documentation and ensure in regulation with Centers for Medicare & Medicaid Services.• Complete referrals to transplant centers; maintain regular contact with transplant centers regarding patient status. If a barrier is identified, work with patient and transplant center to address. • Work with patient on financial barriers including insurance and prescription coverage, American Kidney Fund applications, and National Kidney Fund grants.

Greater Boston Area

• Participate in daily rounds with medical team to identify patients with complex needs. Critically assess patient’s hospital course to ensure appropriate services are on-boarded. Refer patients to social work, addiction psychiatry, violence intervention programs, and patient financial services, as needed. Advocate for patients during rounds to ensure patient’s needs are addressed. • Collaborate with multidisciplinary team (medical, physical therapy, care coordination, social work) to develop discharge plan from acute level of care. Review with patient to increase adherence and treatment outcomes. Identify and work to resolve potential barriers such as lack of insurance, transportation, and education.• Complete 48-hour post-discharge follow up calls to assess transition to sub-acute or outpatient level of care. Triage concerns to appropriate team members. Complete patient needs assessment and make appropriate referrals. Follow patients along care continuum to ensure access to healthcare and improve patient outcomes. • Facilitate weekly support group for visitors of currently admitted patients to increase mutual support, empowerment, and access to resources. • Serve as patient/family relations consultant for the Division by working collaboratively with the patient/family and care team. • Oversee Peer Visitor Program by completing six-month post-discharge outreach to survivors to offer support and enrollment in Peer Visitor Program; on-boarding and training peer visitors; managing and providing support to peer visitors; overseeing visits; addressing acute concerns; and completing program evaluation to determine efficacy of program. • Administer screens for Post-Traumatic Stress Disorder using PC-PTSD-5 tool. Provide psychoeducation around trauma-responses and make appropriate referrals to treatment. Provide crisis management to patient’s in acute psychological distress. Collect data on mental health outcomes to inform future interventions.

• Provide case management to clients by conducting psychosocial and strengths and needs assessments at admission, developing individualized treatment goals, facilitating systems meetings to coordinate treatment, critically assessing needs of the client and family to make appropriate referrals for care, and discharge planning including making recommendations for future treatment. • Facilitate weekly family therapy by utilizing short-term solution-focused interventions to stabilize the family and develop skills to be successful post-discharge including behavior modification and safety planning. • Update client’s treatment plans weekly to critically evaluate treatment progress. • Complete live utilization reviews with insurance companies in a timely manner. • Case presentations in clinical rounds with multidisciplinary team (psychiatrist, nursing, interns, and direct care staff). • Advocate for families by working with state and community-based agencies including but not limited to Department of Mental Health, Department of Children and Families, and Department of Developmental Services.• Provide weekly supervision to staff members and bachelor’s level intern. • Provide on call coverage once a month through crisis management interventions for children and families. • Selected by Program Manager to facilitate monthly meetings with regional managers of MBHP, DCF, and DMH. • Developed resource guides for new hires, parents, and patients. • Improved parent satisfaction by developing and facilitating weekly parent support group. • Improved sensory integration for patients by renovating spaces and providing trainings to direct care staff.
