In my last role, I was charged with building relationships with patients, family members, physicians, and hospital staff and marketing facility to boost referrals. I reviewed patient medical records, synthesized clinical information, completed documentation, and delivered clinical reports and treatment recommendations to physicians and key hospital staff. Key Contributions:• Built and cultivated strong trust-based relationships with patients and families, provided education on applicable LTACH level of care, conducted facility tours, and addressed all fears and concerns. • Rounded with physicians, case managers, social workers, and other medical staff, and educated stakeholders regarding PAM's services, positive patient outcomes, program developments, and benefits of referral to PAM.• Performed frequent, informal SWOT analysis of competing LTACHs, and logged all marketing contacts. • Met with Administrators, Case Managers, Directors of Nursing, and other staff at area nursing homes and sub-acute rehabilitation facilities.• Delivered regular reports to PAM and corporate marketing staff regarding marketing activity performance.

In this position, I acted as key liaison between hospitals and Wisconsin Donor Network, and drove marketing efforts aimed at increasing agency visibility and regulatory compliance. I built relationships with physicians, ICU & ED staff, unit managers, hospital administrators, and ancillary departments to maximize referrals. I handled relationships with 17 hospitals, including Aurora, United Health System, and Wheaton Franciscan Health Systems. As a manager, I supervised, trained, and evaluated a team of 4 direct reports while driving strategy and team efforts, procedural development, resource allocations, and daily oversight of assessments and referrals. I formed trust-based relationships with patients and family members, conducted clinical assessments, and determined appropriate level of mental health or substance abuse treatment. I also facilitated formal group interventions for patients with substance abuse or mental health diagnoses.
