Current• Provided street-based outreach, navigation, and supportive services to individuals experiencing homelessness• Developed and maintained strong ties to the community, and the Coordinated Entry System (CES), and other homeless service providers• Linked clients to appropriate bridge/interim housing programs and assist them in completing required paperwork to enroll them in permanent housing-related programs• Provided case management to meet the needs of everyone• Built trust and rapport with individuals experiencing homelessness. Assessed homeless individuals utilizing the Vulnerability Index (VI) SPDAT• Linked clients to physical health, mental health, and other supportive services. Enrolled clients in mainstream benefits and obtaining identification (i.e., California Driver’s License, Social Security Card)• Kept highly organized files for each client and entered appropriate data into the region’s Homeless Management Information System (HMIS)• Provided bus tokens, bus passes, taxi vouchers, and/or direct transportation as needed• Participated in case conferencing meetings and other community meetings• Coordinated with other outreach teams in joint outreach efforts
