Current- Provided street-based outreach, navigation, and supportive services to individuals experiencing homelessness
- Developed and maintained strong ties to the community, and the Coordinated Entry System (CES), and other homeless service providers
- Linked clients to appropriate bridge/interim housing programs and assist them in completing required paperwork to enroll them in permanent housing-related programs
- Provided case management to meet the needs of everyone
- Built trust and rapport with individuals experiencing homelessness. Assessed homeless individuals utilizing the Vulnerability Index (VI) SPDAT
- Linked clients to physical health, mental health, and other supportive services. Enrolled clients in mainstream benefits and obtaining identification (i.e., California Driver’s License, Social Security Card)
