Initial trial cohort had 100% reductions in re-admission. Broader implementation reduced re-admission by 60%. Program is designed to be modifiable and scalable for other chronic disease states: pneumonia, COPD, MI, post-surgical/orthopedics care.Developed LVAD inpatient and outpatient support programs/protocols to facilitate expansion of the Cardiac Rehab program to include LVAD patient population.
