– Implementing Conflict Free Case Management which was made mandatory by Centers for Medicaid and Medicare in 2014– Staff Authority over Conflict free case management and case management redesign within the Department – Developing a new case management model in the state of Colorado which includes program modifications, process changes, rate methodology.

– Provided technical assistance to providers of eleven waivers to come into compliance with federal regulations. – Provide technical assistance to provider agencies regarding the final settings rule– Educated individuals and agencies on person centeredness – Responded to written and verbal input from providers, other team members, clients, and stakeholders.

– Administered, reviewed applications and utilization of principles and standards pertaining to operations– Facilitated stakeholder engagement meetings– Recommended and integrated efficient operational practices, identify and address operational and process gaps– Solicited participation of stakeholders for the Hospital Back Up (HBU) redesign efforts–.

– Managed a caseload ranging from 50-75 Clients per month – Assessed the functional eligibility of adult individuals who are elderly, or have mental or physical disabilities, and evaluated the need for long term services and support (LTSS) via Medicaid (LTC) program waivers.– Liaised between client and referral sources including: counties, hospitals.
