- Non-profit organization traveling to Peru to perform cardiac surgery on children with congenital heart defects. - Travel to remote part of the country and performed physical and dental assessments on children. - Monitor cardiac and respiratory status after procedure with sedation was completed.- Educate parents and children on importance of proper dental hygiene.

Kidney Transplant

- Coordinate care of high acuity patients and identify needs of patient/family. Provided high quality care to critically ill patients in cardiac, respiratory or renal compromise,including those requiring surgical intervention.

- Use protocols and pathways in line with established disease management and care management programs to optimize clinical outcomes.- Demonstrates a thorough understanding of the cost consequences resulting from Care Management decisions through use of Health Plan Benefits, Division of Financial Responsibility (DOFR), utilization of metrics and CM reports.

- Develop and implement agency wound care program to include patient care protocols and documentation tools. - Perform home visits and/or telephone consults as per referral criteria. - Preform head to toe skin assessment to determine skin condition and document.- Document wound conditions, stage of healing and provide wound care when necessary. - Educate patient and families on wound care and pressure ulcer prevention strategies.
