The Manager of Quality Informatics is responsible for managing Healthcare Effectiveness Data and Information Set (HEDIS) planning, data collection, quality control, analysis, and reporting to meet all requirements of National Committee for Quality Assurance (NCQA) and other regulatory agencies (DHCS, DMHC, CMS). The Manager of Quality Informatics applies both business knowledge and technical expertise to support HEDIS strategies, goals, and initiatives. The Manager of Quality Informatics is responsible for generating and validating IEHP’s HEDIS rates and serving as the technical lead on all HEDIS audit and submission processes. The Manager of Quality Informatics supervises the HEDIS Reporting staff in data collection, data validation, monitoring and reporting for all aspects of HEDIS reporting and improvement activities.

They are responsible for working with practices to facilitate redesign efforts, support care teams, introduce population health management and support achievement towards improved process, clinical outcomes, and value-based care. Practice coaches have primary responsibility for practice coaching within Health Homes Program external (Model 1) sites.

The Data Analyst Manager is responsible for formulating management techniques for data quality collection to ensure adequacy, accuracy, and legitimacy of data. Supports the CITO by creating and enforcing policies for effective data management and serves as the liaison between SACHS departments' data needs and the Information Services reporting team. Responsible for final stated organizational, and departmental evaluation and outcome tracking and reporting.

The MSW Case Manager, funded by First 5 San Bernardino, provides intensive case management services to SAC Health System patients. Duties include community needs assessment creation, distribution and analysis, identification of key community resources in the surrounding area, and behavioral interventions to empower patients in self-management of community resources.
