• Develop a five-year plan based on sound morbidity / morality injury data and evidence-based or promising innovative strategies.• Manages the Cooperative Agreement to ensure the program is within compliance with established standards, contracted goals and objectives.• Reviews and analyzes information and data concerning IP program objectives, to determine effectiveness; makes recommendations for changes in interventions.• Develop partnerships through leading or participating in a multidisciplinary injury prevention coalition to share resources, expertise, and collaborate in planning, implementing and evaluating projects.• Develop program budget to program specific objectives, activities and administration needs annually. • Complete and submit, monthly, semi-annual, and annual reports to Indian Health Services.

• Manages the CHR program daily functions, scheduling and administrative functions.• Provide quality healthcare services, health promotion and disease prevention education to American Indians within the county. • Plans and implements CHR Program health screenings, health outreach activities, home visits.• Assess the community healthcare resources, both Indian Health Services and non-IHS agencies to ensure the best services are made available to the American Indian Community.• To ensure compliance with the requirements of the CHR data collection, and reports are completed utilizing the Resource Patient Management System (RPMS).
