This administrative role consisted of implementing communication , training and outreach activities for organizations programs. Role Details : 1. Assisted the unhoused population to obtain resources , shelter and housing opportunities through the CalAim program . 1. Intense case management with clients to help them obtain vital documents. 2. Worked along side clients to complete EHV (Emergency Housing Voucher) application to submit to back to housing authorities. 3. Provided necessary resources for clients that were in need of food, government benefits, and employment opportunities. 4. Accompanied clients to view apartments or SRO for rent. 5. Worked with clients PCP’s to connect clients to different housing opportunities depending on disability or age . 6. Entered any changes ,case updates and notes into HMIS and KIPU software system .

Provided street outreach services to unhoused individuals that were disabled, mentally disabled or experiencing primary , secondary or tertiary homelessness. 1. Met with clients in South OC cities to conduct intakes and VI-SPDAT assessments. 2. Worked alongside HLO (Homeless Liaison Officer) to provide assistance to clients in need of shelter or substance rehab. 3. Attended match meeting for clients on CES(Coordinated Entry System) 4. Knowledge of process for MHSA Certification for particular unhoused individuals that are eligible. 10. Trained in Crisis Intervention .11. Lead and trained new outreach coordinators on role duties, documentation, client engagement and more.
