- Led the design of new care model for medically complex seniors with a strong focus on managing advanced chronic conditions, delivering high-quality palliative care interventions, and identifying non-medical factors that drive avoidable healthcare utilization. - Partner with Product to develop and implement processes, tools, and trainings to drive practice consistency and clinical outcomes for new service model - Create and disseminate clinical best practices relevant to a medically complex population of older adults. - Develop and delivered communication training to over eighty professionals from diverse clinical backgrounds. - Lead a team of advance practice providers and physicians providing clinical supervision, coaching and mentoring. - Inform the development of predictive algorithms to identify high-risk patients to target for clinical interventions.- Treat members on a weekly basis who often had advanced illnesses and limited life expectancy providing compassionate and evidence-based treatments to maximize quality and quantity of life without prolonging suffering.

- Completed palliative medicine consultations in inpatient, outpatient, and post-acute settings across four major health systems in the Twin Cities addressing symptom management, leading care conferences and facilitating goal-concordant care. - Successfully designed, secured funding, and completed a research protocol for an observational research study assessing the association between prescriptions for durable medical equipment and future healthcare utilization.- Completed requirements for Board Certification in Hospice and Palliative Medicine

- Designed and implemented a clinical / operational strategy that improved the completion of advance care planning documentation across various clinical departments including family medicine, cardiology, pulmonology, and hospital medicine.- Led an interdisciplinary team of social workers, nurses, and providers responsible for completing advance care planning discussions. - Conducted research projects evaluating over-treatment of older adults with type 2 diabetes and published findings in JAMA Internal Medicine. - Served as inpatient faculty supervising family medicine residents during hospital rotations. - Treated hundreds of medically complex adults admitted to Kaiser Permanente hospitals performing diagnostic and therapeutic interventions, care coordination, and discharge planning

I worked at Johns Hopkins University School of Medicine's Evidence-based Practice Center. As a research assistant, I created tools for data abstraction and directed data analysis for various systematic reviews; managed communication among a team of physicians, researchers and government agencies; and contributed to briefs on existing scientific literature for the Agency for Healthcare Research and Quality.

As the Arline and David L. Bittker Fellow at Hillel’s Schusterman International Center, I helped supervise a national internship on 19 campuses that included over 60 college interns; led workshops on group dynamics and teambuilding during national training conferences; and managed grant intake, review, and notification for a $50,000 grant pool for professional projects.
