Plan and implement strategies in order that Knowsley residents can access individualised health improvement advice. To develop methods for effective delivery through primary care settings and creating service linkages with local, community and voluntary sectors.To liaise with other managers and programme leads from within and outside of the borough, to coordinate marketing and promotion of the service to ensure referrals are appropriate and develop guidance and share good practice with others.Contribute to the PCT's and MBC Public Health priorities and targets by building sustained lifestyle and behaviour change amongst target groups and facilitating training to the specified level of competencies required for all staff.Support the implementation and development of an electronic system to record service activity and outcomes and to support other departments and programmes to meet external and internal performance management requirements.Develop and coordinate a partnership group to ensure the programme meets the objectives of the commissioners using existing tools such as health equality audit to devise and implement performance measures and evaluations.Advise the partnership group about strategic issues and to facilitate the managers responsible to coordinate a common approach to operational issues.Coordinate and analyse financial systems and information and provide regular reports to senior management and directors ensuring service risks, barriers and issues are identified and ensure contingency plans are agreed through the partnership group. I worked for Crewe and Nantwich Borough Council for a number of years as a member of the Health Development Team, fulfilling various roles of Health Project Administrator, Exercise Referral Specialist, Health Walk Coordinator, Weight Management Advisor and finally Health Project Coordinator. The teams focus was healthy lifestyle support implementing a range of projects and initiatives to address the health and social needs of the boroughs residents in accordance with health related targets of the Borough Councils Community and Corporate Strategies and Health Policy.

Visit students housed within various placements throughout Widnes and Runcorn on Health and Social Care programmes of study and confer with both the students and placement supervisors on there progress.To deal with and report any complaints about the students or their placements tactfully and professionally, Meeting both students and supervisors separately and together to assess accurately issues and suggest solutions within the boundaries of the college,Write progress plans and keep accurate records of visits and progress of students to report back via the college protocols to lecturers on issues, progress and possible solutions.Assist students to complete their placements to the best of their abilities and to ensure both students and placements are in agreement as to working conditions and responsibilities, ensure protocols and polices are adhered to both by students and placement supervisors
